Description

The most popular option for getting started is the Complete Assessment Package, and is designed for those with routine clinical conditions, such as type 2 diabetes, high blood pressure or high cholesterol.

This package provides you with a custom-designed Meal Plan, as well as detailed Nutrition Education so that you can begin using your Meal Plan to achieve your health and weight goals.

The Complete Assessment Package begins with an initial assessment appointment where I will meet with you to collect detailed information about your health and risk factors, including your personal medical history, family medical history, and dietary and lifestyle information. Lifestyle questions include what you like to eat and when, food likes and dislikes, religious, cultural, or ethical dietary restrictions, and other factors that impact your health and weight, such as sleep habits and activity level.

Based on the information collected during the assessment visit, I will design a Meal Plan to help you accomplish your goals.

The Nutrition Education teaching explains the different categories of foods, the serving sizes, and number of portions to eat at each meal, as well as how to make food exchanges within each category of food.  It walks you through how to implement your Meal Plan, so all you need to do is decide is what to eat.

To get started,  please download the Intake and Service Option Form* (available under the Services tab), complete it on your computer, then email it to the address on the form.

*If you would like a password-protected form, please send a note using the Contact Me form on the tab above.

This package serves as a basis for the Dietary Management Package which can be taken should you want some longer term follow up or coaching, as you implement your lifestyle changes.
