Book An Appointment
Getting started with hourly services or any of our packages is as easy as 1-2-3.
1. Download the current Intake and Service Option Form, which outlines the services we provide and their costs (the current form is dated November 15, 2025. It has been reformatted, and prices remain unchanged).
If you would like a password-protected form, please send us a note using the Contact Form, and we will send you one.
2. Complete the Intake and Service Option Form on your computer.
If you are using a Windows computer, please complete the form using the free Adobe Acrobat online fill and sign tool. If you are using a Mac, please complete the form using Preview.
Since this form dovetails with our accounting and charting system, we require the form back “as is” – not scanned, and not electronically signed (which locks the document, making it unusable from our end). Please type your name in the signature box.

3. Email the completed Intake and Service Option Form
Please send the completed Intake and Service Option Form, along with a downloaded PDF report of your most recent lab test results, to the email address on the first page of the form.
Copies of lab work need to be received before booking services, but don’t worry if you don’t have all of these results. Please send what is available.
