To book an appointment, please send in your completed current Intake and Service Option Form (available here) to the email address on the form, along with your availability for appointments, and I will send you a choice of appointments.
Please note the current form is dated April 20, 2023 — and while prices continue to remain the same, services that are currently available and their descriptions are on this form.
PAYMENT OPTIONS
Payment for services can be made by credit card in the secure online Shop, or by sending an e-transfer which saves the credit card charges. An invoice can be provided first to make it easier for those paying by e-transfer.
If you would like to use the Flexible Payment Option, please be sure to download the Flexible Payment Option Form and send it to the email address on the form, along with your completed Intake and Service Option Form.
For scheduling purposes, prepayment for an appointment is required at the time of booking; however, cancellations or rescheduling is available with 24-hours notice, as well as in the event of an unforeseen emergency.

- Download the Intake and Service Option Form to your desktop, fill it out on your computer* (not by hand, please) selecting your services. Please send a note using the Contact Me form if you would like a password-protected form.
- Email the completed form and available recent blood test results to the email address on the form.
- Let us know your availability and book your appointment.
* If you don’t already have it, you can download the free Adobe Acrobat Reader here to complete the pdf form on a Windows-based computer. If you are on another type of computer, a phone or a tablet, then you can use Adobe Acrobat online fill and sign tools.
