BOOKING AN APPOINTMENT
I don’t use a booking app because different types of visits require different length appointments. When you send your intake documents, please let me know your availability within my hours (weekdays 9:00 AM to 5:00 PM, 9:00 AM to 3:00 PM Fridays), and I will send you a choice of appointments.
Some people prefer to pay for packages at one time and get an updated receipt at their last appointment with all the dates and times of services for submission to their extended benefit plans. Others prefer to use the Flexible Payment Option which enables them to pay for each service in a package as they go. Feel free to use whichever payment option suits you best.
If you would like to use the Flexible Payment Option, please be sure to download the Flexible Payment Option Form and send it to the email address on the form, along with your completed Intake and Service Option form.
Payment for services can be made by credit card in the secure online Shop, or by sending an e-transfer which saves the credit card charges. We would be happy to send an invoice first to make it easier for those paying by e-transfer.
For scheduling purposes, prepayment for an appointment is required at the time of booking; however, cancellations or rescheduling is available with 24-hours notice, as well as in the event of an unforeseen emergency.

- Download the Intake and Service Option Form to your desktop, fill it out on your computer* (not by hand, please) selecting your services. Please send a note if you would like a password-protected form.
- Email the completed pdf form and any available recent blood test results to the email address on the form.
- Let us know your availability and book your appointment.
* NOTE: If you don’t already have it, you can download the free Adobe Acrobat Reader here to complete the pdf form on a Windows-based computer. If you are on another type of computer, a phone or a tablet, then you can use Adobe Acrobat online fill and sign tools.
