NOTE: this is for paying for services via credit card. To save the credit card charges, you can send an e-transfer when you book an appointment . For your security, the e-transfer will be automatically deposited. I would be glad to send you an invoice with the tax already calculated to make it easy.


 

Hourly Consultations are a great option for those who would like to meet me to discuss their needs, or the needs of a family member and to see if I am a ”good fit”.

Hourly Services are also ideal for taking the same services available as a packages, but billed on an hourly basis (as required by some extended benefits plans).

Step-by-Step instructions for Booking Your Appointment:

As easy as 1-2-3

    1. Download the Intake and Service Option Form (available under the Services tab) to your desktop, and fill it out on your computer* (not by hand, please).
    2. Email the completed pdf form, any available recent blood test results to the email address on the form.
    3. Pay for your services below via credit card or send an e-transfer and save the credit card charges

* NOTE: If you don’t already have it, you can download the free Adobe Acrobat Reader here to complete the pdf form on a Windows-based computer. If you are on another type of computer, a phone or a tablet, then you can use Adobe Acrobat online fill and sign tools.

For those who prefer to meet with me first to discuss their needs, share details of their health background, or see if I am a ‘good fit’, the Hourly Consultation is the place to get started.

If you decide to take a Complete Assessment Package or one of the other specialty packages while meeting with me, any remaining time in your initial Hourly Consultation can be ‘banked’ and used later for follow-up services.
