Flexible Payment Option A – for the Complete Assessment Package (first visit)

To take advantage of the Flexible Payment Option when taking the Complete Assessment Package, please download and complete the form available here, return it to the office and then pay for your first visit.

Step-by-Step instructions on paying on the secure server;

    1. click “add to cart”
    2. once it has been added, you will see a turquoise band at the top of the screen indicating that the item was added to your cart.
    3. If you are ordering more than one service / package, add the additional services now.
    4. Once you’ve ordered the services you want, click on “view cart” (on the turquoise band at the top) and you will see where to pay.

Description

To take advantage of the Flexible Payment Option when taking the Complete Assessment Package is also as easy as 1-2-3. The instructions below are for paying for your first visit (the Assessment appointment).

Remember, the first step (as described under the Complete Assessment Package) is to download and complete the Intake and Service Option Form available and return it to me at the email address on the form, along with a copy of your last blood test results and the times you are available for an appointment.

When paying via credit card please;

  1. Download and complete the Flexible Payment Option Form available here
  2. Return the completed form to the office via email to the address on the form
  3. Pay for the first visit by adding this item to your ‘cart’ and select credit card.

Note: GST (5%) is included in the cost of services.


When paying via e-transfer please;

  1. Download and complete the Flexible Payment Option Form available here
  2. Return the completed form to the office via email.
  3. An invoice will be sent to you. Note:  GST (5%) will be added to the cost of services.
  4. Pay for the first visit by sending the e-transfer to the office with the security word provided.

Note: If paying via e-transfer please do not add this item to your ‘cart’.
