- Download the Intake and Service Option Form (available under the Services tab) to your desktop, and fill it out on your computer* (not by hand, please). Select the Specialty Hourly Service.
- Email the completed pdf form, any available recent blood test results to the email address on the form.
- Pay for your services below via credit card or send an e-transfer and save the credit card charges
*You can download the free Adobe Acrobat Reader to complete the pdf form, if you don’t already have it on your computer.
